Dhiothu drops knuckleblades. So the necklace allows an option to use those weapons at endgame. Unfortunately, only rangers and rogues can use those.

Pretty sure only rogues gonna use them. It might be good for an auto build, not sure if you can use all rogue skills with a knuckleblade. In any case, they seem pretty cool to me.
