Forgot Username and Password

Anyone here who can help me get my account back? I forgot my login username and password. I stopped playing around 4 months ago. Now I want to play the game again. I have character in epona named "Genjiro". Before I stopped playing I was in a clan named Pantheon. My character is a level 147 ranger.
